The Inter-Agency Referral System is a secure web-based platform designed to help organizations coordinate referrals, connect individuals with appropriate services and monitor referral progress in a structured and timely way. It provides a shared digital process for sending, receiving, reviewing and following up on referrals between authorized agencies.
Organizations often need to refer individuals or households to other agencies when they require services that are not available within the referring organization. These services may include protection, health care, education, legal assistance, psychosocial support, shelter or livelihood support.
When referrals are managed through paper forms, phone calls, emails or separate spreadsheets, information can become difficult to track. Staff may not know whether a referral has been received, accepted, completed or closed.
The Inter-Agency Referral System was developed to improve this process by creating a centralized and organized referral workflow. It helps participating organizations coordinate more effectively while maintaining accountability and protecting sensitive information.
The system allows authorized staff to create a referral request by recording the necessary client information, required service, referral reason and receiving organization.
After a referral is submitted, the receiving organization can review the request and update its status. Staff can record actions taken, follow-up information, service outcomes and closure details.
The referring organization can then monitor the referral’s progress without relying on repeated phone calls, emails or manual updates.
Authorized users can create a new referral and enter the information required by the receiving organization. The referral can include the requested service, level of urgency, reason for referral and relevant supporting details.
The system connects participating organizations through a shared referral process. Each referral is directed to the agency responsible for providing the requested service.
This helps reduce confusion and ensures that requests are sent to the most appropriate service provider.
Each referral can be monitored throughout its full process. Statuses may include:

Closure notes can also be added to explain the final result and any recommended next steps.

System Administrator – manages the system, participating agencies and user accounts.
Agency Administrator – manages users and referrals within an assigned organization.
Referral Officer – creates, receives and updates referrals.
Reviewer or Supervisor – reviews referral progress and monitors service delivery.
Referral records may contain sensitive personal and case-related information. For this reason, the system is designed with controlled access and secure authentication.
Only authorized users can access referral information. Permissions can be limited based on the user’s organization and assigned role.
The system can also maintain a history of important actions, including who created, received, updated or closed a referral. This improves accountability and reduces unauthorized changes.
Organizations should collect only the information necessary for the referral and follow their data-protection, consent and confidentiality procedures.

I designed and developed the Inter-Agency Referral System as a practical digital solution for organizations that need to coordinate services across multiple agencies.
My work included planning the referral workflow, designing the data structure, developing the user interface, implementing access permissions, creating referral tracking features and organizing dashboard and reporting functions.
This project demonstrates my ability to understand an organizational process and transform it into a secure, practical and user-friendly digital system.
